Transition Strategy Sessions -
These sessions are for you if you are at a crossroads of decision making or are dealing with a crisis. You know something needs to be done but you are not exactly clear what that is. We will put all the pieces together and develop a plan of action. I will make sure you know your options and we will be clear on the choices. I have the time to explain what is happening or facilitate family meetings and help mediate differences. This can make a huge difference in the experience of everyone during this time. I will stay until you are complete and have a plan of action (2-4 hours).
Holistic CARE Sessions –
Mind/Body/Spirit This service shifts your set point. We begin with a strategy session, bereavement or supportive visit, and end with a full body/aromatherapy/Reiki session. This service addresses practical, emotional, and physical needs. It also reduces stress in a very supportive and loving way (3-4 hours).
Family Meeting Facilitator -
There may be times when you would like an impartial person present to facilitate difficult discussions and help various members of the family understand what is presently happening, whether it be the ill persons care goals or what is to come (2-4 hours).
Hospital Advocate -
I will be with you throughout the process for as long as you want. I will make sure you are getting your desired care, understand the information provided, and that you understand your options throughout the process. I will also communicate with your out-of-town family, if desired. We will create a plan based on what you need and want.
Palliative Planning –
This service is appropriate if you are ill or suffering in some way now and need guidance longer than a one-time session: We will develop a plan together based on your needs; it will evolve over time. We usually begin with a one-month plan that will consist of anything from a daily visit, weekly visit/bi-monthly visit, or anything in between. Visits are 2-4 hours. EOL Planning Session This session is for making your end-of-life plans. There may be nothing wrong now, but you want someone who is familiar with you or your loved one and your wishes in your back pocket. You will always know you have someone to call should a situation arise that you need information and to make decisions. (2-4 hours).
End of Life Guide –
More Than a one-time Visit I will guide you during this time and help address all the needs that surface. You are no longer alone trying to figure everything out. We will plan and be proactive in getting your needs met, day by day and week by week. Each visit will be unique as this is a dynamic process and the needs of each visit could be different than the previous visit or the next. We will focus on medical and practical support issues. My expertise is combined with deep listening and spiritual, emotional, and holistic support.
We usually begin with a one-week plan that will consist of anything from a daily visit to every 2 days or whatever is appropriate. Visits are 2-4 hours. Hours of support are planned in this service as an addition to my visits, ex: phone calls, e-mails, research. I do not bill based on an hourly rate unless you request additional services. I may need to refer you to another service provider if I do not have time to accommodate your request.
Special Request Services -
If you need services outside of what I normally perform in the carrying out of my commitment to you, I bill hourly for that. This could include services such as:
· Accompanying to doctor visits
· Extensive research
· Extensive planning
· Caregiver hiring
I will determine if what you are asking is an additional investment.
Phone Consultations –
Deep listening as well as practical suggestions and food for thought to help you get through whatever situation you are in.

IMPORTANT NOTE:
I am based in Sellersville, Pennsylvania and will travel if available. My service is not an emergency service. Continue to use your hospice service for that or your local 911 service. I am always your second call (after the above have been contacted) if you feel you are in an emergency.
